The Medical Histories Management feature offers a handy way for practitioners to document, review, and update patient medical histories, covering everything from past medical conditions and surgeries to family, social, and gynecological histories. This feature ensures that patient history is always recorded in an easy-to-access manner, helping practitioners make well-informed decisions.
The Histories integrate into the patient chart, supporting differential diagnoses, treatment planning, and risk assessment. The History section is populated in a variety of ways from free text boxes for all histories, to history-specific modules to fill in.
Some common tasks you will perform with this feature include:
- View and manage all past and current medical histories in a structured format.
- Sort and filter history by type (Medical, Surgical, Family, Social, Gynecological).
- Track changes, including who updated the history and when.
- Link Histories to patient Encounter Notes, and Consults.
- Archive or modify history items while maintaining an audit trail.
Histories are a key part of preventive care and can be easily created, reviewed, and managed in a few ways:
A. Chart Notes: These can be created and edited during patient visits.
B. Histories Index: This offers a consolidated view of all patient Histories, with options to create, edit, or archive records.
C. Patient Sidebar: Provides quick access to recent Histories during patient interactions.
Use
Below, we will cover all the features of and how to use Histories.
View a Patient's Histories
1. Open the Patient Chart.
2. See the Histories in the right-hand sidebar.
3. Click on Histories sub-tab.
To then view the specifics of a particular History:
5. Click the name of the History to expand it.
Create Histories
Unlike other areas of the Patient record, Histories are created in Chart Notes as opposed to directly in the Issues sub-tab.
1. Open the Patient Chart.
2. Click on Histories sub-tab.
3. Click New History in Charting (you will be re-directed to a new Chart Note with a Histories embed).
4. Enter the information.
5. When you are finished with the chart note, click Sign and Save.
This History will now be attached to this particular chart note, as well as found within the Histories sub-tab.
Edit Histories
1. Open the Patient Chart.
2. Click on Histories sub-tab.
3. Find the History you wish to edit.
4. Click on the name of the History.
5. Click on the Edit icon to the right of the History.
You will be re-directed back to the Chart Notes. You will create an edited chart note here.
6. Make your changes.
7. Click Sign and Save.
You will now see that this History is listed under the Histories sub-tab and when you click on the title you can see it is attached to two notes - the original chart note and the updated chart note. To see what was changed:
8. Click the name of the History to view the information.
9. Click the timeline icon to the right of the History.
Note the details that were changed have been added to the timeline.
Archive Histories
Warning ⚠️: Please be aware this feature is currently not working.
Instead of deleting Histories, Aeon gives you the ability to archive Histories, removing it from active view. These archived Histories can be viewed or restored if desired.
1. Open the Patient Chart.
2. Click on Histories sub-tab.
3. Find the History you wish to edit.
4. Click on the name of the History.
5. Click on the archive icon to the right of the History.
6. View the confirmation message about Archiving the History.
7. Click Confirm.
View Archived Issues
1. Open the Patient Chart.
2. Click on Histories sub-tab.
3. Click the line "Show # Archived Histories" (located under the list of your Histories.
4. Click on the name of the History to see the details.
Restore an Archived Issue
1. Open the Patient Chart.
2. Click on Histories sub-tab.
3. Click the line "Show # Archived Histories" (located under the list of your Histories.
4. Click on the name of the History to see the details.
5. Click the restore icon to the right of the History.
Permissions
- Practitioners: Can view, edit, and manage patient Histories.
- Staff: Can view Histories.
Histories Limitations
These are topics that we are aware of and working to address in the future:
- Patient-submitted history updates for provider review.
- AI-driven history summaries and risk stratification.
Histories Issues
These are current issues that we are aware of:
- Initial Early Access lacks advanced tagging capabilities.
- Terminology standardization across history types requires iterative improvements.
- Management of duplicates are not addressed in Early Access.
