Description
The Enhanced Charting System makes it super easy to document patient medical data, manage tasks, and includes handy features like referrals, lab requests, and prescriptions, all while keeping everything compliant with legal and usability standards. It's designed for practitioners, staff, and other healthcare professionals to smoothly document patient visits, handle tasks, and look up past data. You can use it during patient appointments, chart reviews, and for administrative tasks.
Some common tasks you'll complete in Charting are:
- Create, edit, and finalize chart notes with autosave and versioning
- Utilize embeds forward slash function to add Consults (referrals), Prescriptions, Issues, Histories, Immunizations, and Screenings
- Track changes to drafts, published notes, and linked tasks through the timeline
- Assign tasks and link them to specific chart entries
- Generate and manage medical documents (e.g., referral letters, lab requisitions)
- Sidebar view for a detailed overview of the patient's chart
Location
In the Patients tab, after you click on the name of a patient, click on the Charting sub-tab:
From here you can:
A. Click New Chat Note to start a new chart noteB. Resume a Draft of a previous chart note (if there is one)

Use
Below, we will cover all the features of and how to use Charting.
View a Patient's Chart Notes
1. Click Patients in the left-hand column
2. Find the patient and click on their name
3. Click Charting
You will now see a list of all Chart Notes for the patient. To view a particular note:
4. Click on the title of the Chart Note
If there are any Histories, Issues, Prescriptions, etc that have been linked to the Chart Note, you'll see them noted at the bottom of the Chart Note. If you click on an item, you'll be taken to that item's location to see more details:

Create Chart Notes
1. Click Patients in the left-hand column
2. Find the patient and click on their name
3. Click Charting
4. Click New Chart Note
5. Give the Chart Note a title (*mandatory)
Training Tip 🎓: You are unable to save a Chart Note without a title.
6. Enter your notes in the note area. Feel free to use the formatting options located above the note area
Add New Items
As you enter a Chart Note, you can embed new Tasks, Consults, Screenings, Immunizations, Measurements, Medications, Issues, Prescriptions, and Histories right within the Chart Note so that you don't have to leave the page. To do so:
7. As you are typing, enter the command /task (for example), and you'll see the options that appear:

7. After you choose the type of Task, you'll see a blank embed form appear:

Add Existing Items
Sometimes you want to include previously created Tasks, Consults, Screenings, Immunizations, Measurements, Medications, Issues, Prescriptions, and Histories to the Chart Note.
8. While in the Chart Note, look at the right hand sidebar and see that there are the headings for the areas of the Patient Profile. When you click on the dropdown, you'll see all prior entries:

9. To add one or more of these to the Chart Note, click the + to the right of the item
In this example, I've clicked on the + next to the Issue of Hypertension to add it to the Chart Note:

Then, I've changed the Diagnosis Status from "Suspected" to "Confirmed" along with some notes and saved the Chart Notes:

In Aeon when the Issue is viewed, there is now two Chart Notes linked to this issue and either can be opened and viewed:

When you are Done entering Information
10. When you are done with the Chart Note, click Sign and Save. This will save the Chart Note, and any embeds you've created will now be found in their respective tabs, linked to this Chart Note.
Training Tip 🎓: A key consideration for practitioners is SAVING Chart Notes. Aeon will save as you type. For example, check out this video and watch the Saved ✅ note at the bottom as I type:

Chart Notes in Draft
Aeon allows you to navigate away from a Chart Note in progress and then return to work on it some more.
As you type in a Chart Note, you'll see a display "Saved ✅" at the bottom of the screen, confirming that the information is saved. This happens automatically as you type the note. However, the note is not final until you click Sign and Save.
Here is a note that is in draft. We can see the entry has been saved, but it hasn't been finalized yet:

If you view a list of your Chart Notes, you'll see Draft notes marked as such:

Edit Chart Notes
Due to the importance of Chart Notes, when you edit a Chart Note, you are actually creating a new, updated version of it. The previous version will still be available, and you'll see all the changes noted in the timeline. You can do two things with saved Chart Notes: Strikeout Previous Text and Add New Text.
1. Locate the Chart Note you wish to edit

Link a Calendar Appointment to a Chart Note
You can link the Calendar Appointment from within the Chart Note, or from within the Chart Note you can link the Calendar Appointment. See the complete steps here.
Permissions
- Practitioners: Full access to create, edit, and manage chart notes, embeds, and linked tasks
- Staff: Viewing only for Chart Notes
Limitations
These are topics that we are aware of and working to address in the future:
- Fully automated updates to linked tasks when changes occur in charts
- Real-time collaboration for charting across multiple users.
- AI note-taking
- Voice-to-text direct integration
Issues
These are current issues that we are aware of:
- Autosave may require optimization for high-volume users to avoid latency
- AI note-taking is not yet part of the app
